When you arrive at Little Creek Veterinary Clinic, you can expect to be greeted by our friendly front-end staff, who will confirm your information on file is correct and then continue the check-in process. We will begin by weighing your pet and accurately update your pet’s file.

If you would like us to request your pet’s records from a previous veterinarian, please complete the Medical Records Request Consent Form.
MEDICAL RECORD REQUEST CONSENT FORM
An important part of your pet’s health is making sure their medical history is current. If you have been to another veterinary clinic, we ask that you please have the complete medical history forwarded to us so we can attach to their patient file here. That way, we have a record of any vaccinations, previous surgeries or other relevant medical history. Please note that we cannot contact your previous veterinary clinic to have them send records here, legally the request must come from you. We ask that the complete medical history be sent to us as info@littlecreekvet.ca before your first appointment so that our veterinary team can be up to date on your pet’s health.
