International Travel Appointment Form

This form MUST be filled out PRIOR to your appointment. Please DO NOT fill out this form if you are not an established client at Little Creek. Please give us a call or fill out our new client form to establish your file.

 

The staff at Little Creek Veterinary Clinic are not responsible for providing international importation paperwork.

 

If you are travelling with your pet, we recommend contacting a pet travel agency, and/or the embassy of the country you are entering. They will have the most up to date travel requirements.

At the time of your visit, we will provide you with an international health cetificate, rabies certificate, and we can assist you in filling out the required importation documents. Please be sure to forward any documentation that needs to be filled out PRIOR to your appointment. Any requirements for vaccines, titres, deworming, or external parasite control should be emailed prior. This can be emailed to info@littlecreekvet.ca.

 

Any documentation required on legal size paper must be brought pre printed to your appointment. We are unable to print these documents in clinic.

Are you aware if an exam is required prior to departure? *
Do you have the required documentation for the country in which you are travelling to? *
Has your pet been vaccinated at another veterinary clinic within the last 3 years? *

 

If your pet has received a rabies vaccination at another veterinary clinic, said clinic will be responsible for providing you with a rabies vaccination certificate.

 

We CANNOT provide a certificate for a vaccination we did not administer.
